HEALTH SERVICES
The Health Office for Pre-K through 12th grade is located in the Elementary Office area. We look forward to meeting your child's health care needs and supporting their education at Triton.
If your child has any changes in their health status, please let the Health Office know as soon as possible so we know how to better help your child if the need arises. It is important that the "Health Information Form" be completed thoroughly and returned to the Health Office at the beginning of the school year.

Hello Triton Community! Nurse Andrea and Tessa are ready for another year working to keep the students safe, healthy, and engaged in the classroom. Please explore the page for medication forms, immunization forms, and some simple guidelines on when to keep your student home. We are always happy to chat with parents about any questions or concerns that come up. We want to do our very best to provide quality care here at school and work together with you to have healthy kids learning in the classroom. IMMUNIZATIONS
State Health Laws and School Required Shots
Immunizations are required before the first day of school in order to attend school in Minnesota. Students must show proof of these immunizations or file a legal exemption with the school before attendance. Medical exemption and conscientious exemption forms must be signed by a parent/guardian and be notarized.
Schools will no longer be accepting the parent/guardian's statement that the student has had chickenpox. Your medical doctor needs to sign a form saying that the student has already had chickenpox disease.
